Physician Care Coordination: Referrals, Consults, and Transitions of Care
Bayou Desiard Country Club at 6:30
Today it takes a team of physicians and other practitioners to provide comprehensive care for a patient. The timely and accurate communication of information is frequently challenging between providers of care. These challenges to the flow of information have the potential to impact the quality of care for patients and are a major factor in claims of medical professional liability.
This presentation will illuminate the key problem areas (especially those related to referrals, consultations, and transitions of care), how the patient's perception of care may be affected, and strategies to overcome the identified challenges and mitigate risk.

Time: 8:00 am - 1:00 pm Hilton Garden Inn West Monroe (Salon A&B) Description: In January of 2016 the CMS Administrator Mr. Andy Slavitt announced a sweeping change to the Medicare Incentive Payment Program and Meaningful Use (MU). The changes are to involve legislation recently passed, e.g. the “Medicare Access & CHIP Reauthorization Act of 2015 (MACRA)”. This Act provided for two new methods of physician Medicare payment. Those were a “Merit-Based Incentive Payment System (MIPS)” and “Alternative Payment Models (APMs)” as part of a “Quality Payment Program”. MU was involved in the former MIPS, i.e. the MIPS combines parts of the past Physician Quality Reporting System (PQRS), the past Value-based Payment Modifier system, and the past Medicare Electronic Health Record (EHR) incentive program. There will now be one single program under which physicians as “Eligible Clinicians (ECs)” will be measured. The areas of measurement will include Quality, Cost & Use of resources, Advancing Care Information (similar to current MU), and of great importance Clinical Practice Improvement. Under these changes physicians as Eligible Professionals (EP) will see positive or negative reimbursement adjustments under MIPS starting in 2019, based on their 2017 performance. The 2017 lecture series will explain the origin of these changes, the application to every physician accepting federal Medicare payment, and will describe individually the areas MIPS and APMs. Presenter: Kenneth E. Rhea, M.D., FASHRM Time: 6:00 - 7:00 pm St. Francis Medical Center (Main Auditorium) The OMS Oyster Party will be held on
