Physician Care Coordination: Referrals, Consults and Transitions of Care

Today it takes a team of physicians and other practitioners to provide comprehensive care for a patient. The timely and accurate communication of information is frequently challenging between providers of care. These challenges to the flow of information have the potential to impact the quality of care for patients and are a major factor in claims of medical professional liability.

This presentation will illuminate the key problem areas (especially those related to referrals, consultations, and transitions of care), how the patient's perception of care may be affected, and strategies to overcome the identified challenges and mitigate risk.

Presenter: Kenneth E. Rhea, M.D., FASHRM

Dr. Rhea received his engineering degree from GM Institute of Technology in Flint, Michigan, his Bachelor of Arts degree from Westminster College in Fulton, Missouri, and his Doctorate in Medicine degree from Vanderbilt University School of Medicine in Nashville, Tennessee. He completed surgical internship at Indiana University Medical Center and residencies in neurosurgery, ophthalmology, and associated neuropathology research. He served in the United States Navy.

In private practice, Dr. Rhea was Director of Medicine and Surgery at Texas Medical Care, Deputy and Medical Consul­tant to Harris County Sheriffs Department in Houston, and Director of Clinical Ophthalmology for South Texas Eye Physi­cians.
